We are a PPO provider, we do not accept HMO/DHMO/COMMUNITY CARE/AHCCCS/ OR MEDICAID ONLY PPO PLANS. As a courtesy, our office will bill to any Traditional Indemnity or PPO plan. We are not an HMO or DMO contracted office. Our office obtains a verbal estimate of benefits from your Insurance Carrier. A Treatment Plan & Estimate will be created for you. Estimates are never a guarantee of payment. Your estimated copay is due at the time of service. If there is any additional balance after your insurance remits payment, a statement will be created and sent to you. Your payment will be appreciated within 10 days.
